Simply put, PCMS significantly improves the overall quality of healthcare while dramatically reducing its overall cost. The Patient Centric Medical System (PCMS) provides an Integrated Healthcare Record (IHR) to the individual, which contains up to date medical information at the point of service, Unlike PHRs, EHRs, or EMRs contained in a multitude of diverse systems, that will never be used anywhere other than the place the healthcare information is contained.
1HIE developed a portable medical information solution. The Patient Centric Medical Solution, PCMS, an Integrated Healthcare Record (IHR) that provides a “Health Information Exchange of One”. PCMS provides individuals their current, certified, secure medical record that is owned and controlled by them. PCMS is in a cloud environment that manages, classifies, protects and control valuable digital content and data, enabling the information collaboration across multiple enterprises in a secure and controlled manner. PCMS features tokens and is the first platform designed to work as both for identification and a key to sophisticated cloud-based services. 1HIE is currently deploying it for selective Medicaid, Telehealth, and Concierge Medical opportunities our community.
Chronic disease in the underserved and the elderly with Multiple Chronic Conditions (MCC) is estimated to account for over 84% of annual healthcare spending in the United States, with many of these preventable costs expected to rise as the population continues to age.

The “1HIE Digital Solution” provides electronic tracking and certification capability of all transactions within this process, and additional measurements, therefore, meeting all CMS billing guidelines. The “1HIE Digital Solution” center provides services to beneficiaries that can have up to 5 chronic conditions. “The 1HIE Digital Solution” creates a healthcare record that can be remotely monitored and provides an alerting functionality to existing healthcare systems. The “1HIE Digital Solution” significantly reduces ER and readmissions by up to 60%.
The elderly and underserved population of the United States accounts for a tremendous percentage of the country’s healthcare expenditures. The United States spends twice as much on healthcare as do the rest of the world’s developed nations. In fact, 18% of our gross domestic product (GDP) each year goes to support the country’s healthcare systems. The percentage of the U.S. economy spent on healthcare for Medicaid beneficiaries and individuals ages 65 and older exceeds 5% of GDP — a proportion that is expected to double by 2030 and triple by 2050, reaching a staggering 10% to 15%, an unsustainable amount.
If this problem is not solved, the United States will be forced into a National Healthcare System that will have to ration healthcare as is done by so many nations where innovation in healthcare is non-existent.
The problem is simple to solve: use the “1HIE Digital Solution” – a Remote Patient Monitoringservice that authenticates each healthcare monitoring transaction and transmits data from the patient to a healthcare professional in a different location that accesses the data for potential intervention though advanced alerts algorithms.
The “1HIE Digital Solution’s” Remote Patient Monitoring Centers initially collects five objective physiological biomarkers (temperature, heart rate, blood oxygen saturation, blood pressure and weight), subjective reporting of the number of chronic health conditions (independently verified). In addition, a number of lifestyle questions are discussed (e.g.: Have you taken your medications? How hard is it for you to stand up, etc.). This authenticated data is transmitted to a secure data base and a skilled medical professional will examine each individual’s alerts and take the necessary actions to mitigate the situation. In the second Phase EKG and Glucose monitoring will be added to physiological biomarkers.
“1HIE Digital Solution” is a community-embedded, RPM program targeting low-income and older adults, providing regular assessment of vital signs, monitoring multiple chronic conditions and subjective wellness, and providing wrap-around social services, all of which dramatically reduce the overall cost of care, which is usually not covered by Managed Care Organizations. The “1HIE Digital Solution” reduces the costs to the States and CMS, while improving the beneficiary’s overall healthcare. The “1HIE Digital Solution” is a different RPM program, which is separate from traditional healthcare providers, systems and insurance companies. In other words, it is a different business paradigm that can be extremely profitable to all concerned.
For the billions spent on the Affordable Care Act, one outcome resulted in the fact that the ACT created silos of healthcare information that are fiercely protected by their respective owner. With over 300 EHR systems on the market coupled with the Federal and State regulations, it will be many years for the system to provide an integrated health record (IHR) to the individual.
PCMS APIs that were developed in conjunction with the Missouri Medicaid Passport Program lays the foundation to a totally different approach to interoperability. Since the patient owns their healthcare records, 1HIE created APIs for several EHR applications within the Columbia Missouri area, including the tracking and integration of healthcare records of the Missouri Foster Children Program.
The initial U.S. market for 1HIE's PCMS offerings is approximately $1.3 trillion. This includes the Medicaid, Medicare, Defense Health Agency, Veterans and other federal programs that require interoperability – a sizable market. While this is an incredible fact, 1HIE believes that there are three programs/opportunities that have demonstrated the true value of our approach.
While there are a number of Telehealth Companies in the business of providing RPM, we believe that we are the only RPM/Telehealth Company that serves the Medicaid and elderly poor. This segment has been intentionally overlooked due to its perceived costs and revenue model. In addition, the “1HIE Digital Solution” does not support a point of service healthcare solution, it provides the ability for a group monitoring capability at specific locations (e.g.: community centers, churches, public housing, etc.) providing services other than healthcare. This greatly reduces the cost of providing the “1HIE Digital Solution” and with the use of the new CPT codes, produces a very profitable venture. The companies that provide a similar monitoring service are Managed Care Organizations through their “Case Management Programs”. However, there are several inherent problems with these programs: they mostly conduct telephone interviews and physician conferences, and involve no at home measurements of blood pressure, weight, pulse, oxygen level, or prescription adherence.
